阿肯（黎骏 饰）带着女友玲玲（徐玲玲 饰）以及一干好友前往一座地处偏远的荒岛，花魁准备在那里开一场热闹而又淋漓尽致的派对。然而抵达小岛后，花魁一行人才发现，原来他们并不是岛上唯一的一群游客。咸湿、吹水和山猪（黄秋生 饰）三名古惑仔也来到了小岛上，他们来这里是为了探访在这儿隐居的黑帮老大。黑道的出现让阿肯一行人感受到了不安全，然而，他们却不是真正的问题所在。咸湿和山猪先后被杀，剩下来的吹水找到了阿肯向他们寻求帮助，这意味着，在这座岛屿上，隐藏着一批嗜血而又凶残的杀人狂魔，更糟糕的是，他们的真实身份，是一群麻风病人。
